POSITION SUMMARY - NICUAs a member of the nursing leadership team and in collaboration with other members of the interdisciplinary healthcare team, and in accordance with the NH Nurse Practice Act, all applicable regulatory and specialty standards and EHS policies and procedures, the Clinical Nurse Leader provides peer/horizontal leadership accountable for the clinical nursing management of patient care and the care environment. Maintains detailed knowledge of all patients in caseload and demonstrates excellent communication skills with all members of the interdisciplinary team in order to serve as a lateral integrator of care. Serves as a change agent to translate and integrate evidence into practice; to evaluate and design safe, quality care, and to improve patient outcomes. Maintains 24 hour accountability for care provided by self and others to whom nursing care is delegated.

KEY RESPONSIBILITIESClinician: Designs, coordinates, integrates and evaluates care provided to patients in caseload with particular emphasis on health promotion and risk reduction.
Outcomes Manager: Synthesizes data, information and knowledge to evaluate and achieve optimal patient outcomes. Collaborates with the Center for Clinical Excellence to maintain population/unit level metrics and dashboards; uses metrics with work teams to identify areas of opportunity and facilitates process and patient outcome improvements.
Advocate: Ensures that patients and families are well informed and included in care planning.
Educator: Provides or oversees the provision of education to the patient and family regarding diagnosis, self-care and health maintenance. Collaborates with the Clinical Nurse Specialist and other educators to identify learning opportunities and provide education to clinical staff.
Information Manager: Utilizes Epic and other information systems and technology to improve health care outcomes. Utilizes evidence based practices and the latest research to provide current best practice information to care providers on the unit. Ensures effective communication and flow of information to all appropriate care providers by obtaining appropriate comprehensive patient story and transmitting that effectively to care providers.
Unit Based Analyst/System Analyst/Risk Anticipator: Leads and/or participates in systems review to quality of patient care delivery and at the individual level to critically evaluate and anticipate risks to patient safety with the aim or preventing medical error. Profiles patterns and tailors interventions to meet specific needs of individuals and clinical populations.
Team Manager: Serves as a leader and partner in the interdisciplinary health care team.
Member of a Profession and Life Long Learner: Actively seeks out and integrates new knowledge and skills into the practice setting supporting the change in healthcare and the nursing profession.

EDUCATION/EXPERIENCE/LICENSUREEducation: MSN preparation as a Clinical Nurse Leader required.
Experience: Minimum of three years of experience as a registered nurse in an acute care setting required, two years must be within the applicable area of specialty. Evidence of horizontal leadership experience required.
Certification/Licensure: Current registration in the State of New Hampshire as a Registered Nurse required. Certification as a CNL or achievement as soon as possible when examination criteria are met required. Specialty certification relevant to practice area strongly preferred.
Software/Hardware: MS Office required.
