Position Summary:

Liberty Resources is seeking applicants for a Part Time or Per Diem Reproductive Health Nurse Practitioner. In collaboration with the agency Medical Director, the Nurse Practitioner will function as a member of the Integrated Health Care Team in assisting with a full range of nursing/medical services.

Job Responsibilities:
Reviews and evaluates patients' health histories including obstetric, gynecologic, contraceptive, medical, surgical, family health history, and psycho-social backgrounds;
Performs interviews, health histories, physical examinations, mental status assessments, and diagnostic tests to determine diagnosis and current health status of clients.
Services include examination and screening as well as the full range of birth control options including IUD insertions and removals, STI screening and treatment, pregnancy testing and options counseling, and health education.
Establishes treatment plans, administers medications, and conducts reassessments in conformance with established standards, procedures, and clinical protocols.
Performs clinical assessments for patients initiating PrEP, PEP, and/or HIV treatment.
Participates in the management of clients' medication regimens; prescribes drugs and medication treatments as indicated by diagnosis, lab results and consistent with community standards of care.
Provides all appropriate primary care, care coordination, and medication education to clients and families.
Liberty Resources Clinics are approved sites for the National Health Service Corps Loan Repayment Program (Tier 3).

Qualifications:
Certificate and current registration to practice as a Nurse Practitioner in New York State.
At least four years' experience providing direct services in Reproductive Health Services
Experience with individuals diagnosed with mental illness, or comparable experience is valuable.
Must have eligibility for full and unconditional participation in the Medicaid and Medicare programs. Must possess a valid New York State driver's license and have access to reliable transportation. Effectively use an EMR and complete all required documentation in a timely professional manner.
Must be flexible and oriented to family work.
Sensitive to cultural differences and able to incorporate that understanding into service provision. Regularly required to sit, stand, walk, bend, climb stairs and lift objects of up to 10lbs as required.
