Overview:
We are on a mission to change the way healthcare is delivered and experienced in the obstetrical gynecological provider office setting. Every woman deserves to be treated with dignity and respect in a caring environment with a commitment to their health and well-being. That is why we are looking for individuals that share this same commitment to create a new kind of healthcare delivery for women of all ages. We want to help women navigate the healthcare system in a better and less confusing way to achieve better women's health, maternal and neonatal outcomes. The OB/GYN Case Manager is key to achieving this goal. This case manager role will be responsible for completing risk screenings, assessing psycho-social risk factors, assisting with community connections and referrals, and providing ongoing clinical support and education to women as they move through their care journey. The ideal candidate is caring, compassionate, solution-oriented and committed to providing an outstanding experience and improved outcomes for these women. The candidate must be ready to innovate and be adaptable to a continuously evolving program that will meet the needs of both our providers and patients.
Responsibilities:
Telephonic and/or Face to Face risk screening with developed tools that will assist in identifying risk factors, gaps in care and/or health care resource needsCompleting comprehensive assessments and developing care plans with women and their treating providers.Explain and educate women on their specific conditions, how to identify when to seek urgent/emergent care, and identify barriers to consistent careOngoing follow up to ensure connection to health care referrals, community/health plan resources/benefits and education *includes documentation in appropriate platformParticipate in ongoing interdisciplinary rounds at office and/or health plan level *reserved for high-risk womenFacilitating escalations and provider communication of critical patient needs as determined by clinical care protocols/pathways Additional Job Functions:• Build trusting relationships with providers and patients• Identifying opportunities for improvement w/in workflow and/or external partnerships• Other duties as assigned
Qualifications:
• 2-4 years of case management experience in healthcare and/or provider office setting, women's health experienced required.• An unrestricted TX RN license or compact state license• Ability to work effectively in a collaborative environment to create a best practice care navigation model• Providing feedback to help improve the operational processes, tools and documentation.• Professional written and interpersonal skills when communicating with providers and patients• Ability to prioritize and manage several assigned tasks effectively• Expertise in computer skills, including Office or Google Suite software … EMR experience preferred• Reliable transportation, may have to drive between care centers• Bi-lingual in English and Spanish, preferred
