Maternal Child Health (MCH) Case Manager works with Erie's highest risk pregnant patients and collaborates with medical providers and nursing staff to identify potential patients and assess and deliver supportive services. The MCH Case Managers role includes outreach, advocacy, assessments, care plan, home visitation, and data tracking. The MCH Case Manager is also a mandated reporter to the Department of Children and Family Services. This position requires a self-motivated individual who can manage and take ownership of multiple projects.

  • Provides case management services including coordination of primary care (prenatal and/or pediatric care) and social services, outreach, transitions of care follow up, risk assessments completion, referrals, home visitation, and track clients appointments failures.

  • Collaborates with fellow Case Managers to provide constant floor coverage to assure direct service throughout the day.

  • Displays familiarity and knowledge of the care plan of each client and facilitates referrals and follow-up process.

  • Completes Edinburgh post-partum screenings and Ages and Stages developmental screenings.

  • Collects participants surveys as required by grantors.

  • Maintains quality of the cases by achieving Erie and grantor program goals, including but not limited to, the following areas:

    • Prenatal and post-partum depression and developmental screenings

    • Prenatal and infant face to face visits as well as home visits

    • Individual care plan and assessments

    • Integration with WIC and case management

    • Documenting data into electronic systems (EMR & MHN Connect)
