Job Description
POSITION SUMMARY :

This position is located at the Minneapolis VA Health Care System in the Rehabilitation and Extended Care (REC) Integrated Clinical Community (ICC) as part of the Community Living Center (CLC), unit 1F. CLC is a hospital based transitional care area serving veterans for an extended but limited time. 1F is a rehabilitation unit with nursing staff that include RNs, LPNs, and Nursing Assistants. Common diagnosis treated on unit 1F include but are not limited to: COPD, diabetes mellitus, cancer, congestive heart failure, peripheral vascular disease, depression, decubitus ulcers, amputations, orthopedics and hospice & palliative care services. Services address the full range of functional, physical, cognitive, psychosocial, and leisure needs typically exhibited by individuals requiring rehabilitation. The goals of nursing care and the interdisciplinary team on 1F are: to treat residents and prevent complications associated with disease and disorder; perform diagnostic and therapeutic procedures; educate residents/families to improve self-care abilities; health maintenance; and rehabilitation. The inpatient staff nurse in CLC works rotating tours of duty, including weekends and holidays, to meet unit and resident care needs.

SUPERVISORY CONTROLS :
The CLC Registered Nurse is accountable to the Nurse Manager within the Community Living Center (CLC), unit 1F

QUALIFICATIONS :

Minimum of Associate Degree in Nursing (ADN) or bachelor's degree in Nursing (BSN) is preferred. 1-3 years of experience in rehabilitation

RESPONSIBILITIES/ DUTIES:

a. Utilizes the nursing process in managing, developing, implementing, and evaluating each resident's individual plan of care with the interdisciplinary team to attain specific clinical outcomes.
b. Provides a full range of nursing care to residents with a variety of physical and/or behavioral problems.
c. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others.
d. Participates in quality improvement activities and the evaluation of caregiver issues
e. Assesses learning needs of veterans, caregivers, and staff.
f. Works collaboratively with all levels of health care professional and support staff to facilitate positive resident care outcomes.
g. Effectively communicates with veterans, caregivers, visitors, and members of the interdisciplinary team
h. Guides and direct resident care delivery of support staff
i. Administration & Monitoring of Medications
j. BLS certification is required for this position and can be obtained upon hire
k. Compliance with various Isolation Precautions and Infection Control Guidelines
l. Skills in time management, priority setting, assessment and urgent intervention are essential for this unit
m. Advanced Vascular Access Care and Maintenance
n. Maintains prevention & management of disruptive behavior skills, can be trained upon hire
o. Patient and caregiver education and support.
p. Participation in High Reliability Organization (HRO) & professional governance initiatives
q. Promotes evidence-based best practices and nurse sensitive indicators (including but not limited to Falls Prevention, Hospital Associated Infection Prevention, Medication error prevention, Safe Patient Handling, and Skin Safety)
