JOB TITLE: Community Living Coach II

DIVISION: Residential

SUPERVISOR: Community Living Manager II


JOB SUMMARY: Provide direct care, supervision, and training for people with disabilities residing in a residential facility.

EDUCATION OR TRAINING REQUIREMENT: High School diploma or equivalent;

EXPERIENCE REQUIREMENT: 1-2 years working with individuals with disabilities; 1 year experience working with children and/or young adults with disabilities and/or related field preferred.

SPECIAL LICENSES, REGISTRATION OR CERTIFICATION: Valid SC Drivers License with safe driving record



I. Training/Program Responsibilities

  • Implements care, training and supervision as outlined in the individual's residential support plan.
  • Assists individuals in participating activities of his/her choice to support community integration.
  • Follows agency protocol to report all emergency/crisis situations appropriately to protect the life, safety, and well being of individuals supported.
  • Provides transportation and supervision for individuals as required to meet employment, recreational and leisure activities.
  • Ensures that all information regarding daily activities, problems and needs of residents are recorded properly according to agency practices.
  • Ensures each individual's personal hygiene needs are met.
  • Provides training and assistance in choosing appropriate apparel for the weather, activity, or event to ensure comfort and well being of the individuals
  • Assist with meal preparation, laundry and household cleaning as needed.
  • Utilize safe lifting techniques as instructed in training using appropriate body mechanics to lift, move, and position individuals.
  • Follows the individuals approved behavior support plan to proactively address challenging behavior.
  • Abides by code of ethics and serves as a positive role model for individuals.
  • Develop and maintain positive and productive relationship with individuals, families, co-workers, and community members
  • Maintain objectivity in position in order to set appropriate limits while working with individuals.
  • Support individuals in their personal growth and development, respecting cultural, ethnic, spiritual and individual differences.

  • II. Responsibilities Relating to Health and Safety of the Individuals Being Served
  • Ensure that the home is safe, sanitary, and free from obvious hazards at all times.
  • Corrects any health and safety hazards immediately and/or report by phone or appropriate systems.
  • Perform housekeeping responsibilities to include but not limited to, kitchens, bathrooms, windows, bedrooms, and floors.
  • Perform visual safety inspections and cleaning of agency vehicles as needed on each shift.
  • Conducts evacuation drills, as directed by supervisor. Records results of each drill.
  • Follows agency protocol for all potential emergencies and disasters. Maintains documentation as required.
  • Follows policies and procedures for all medical emergencies.
  • Ensure that food is properly stored, labeled as required, and protected from contamination and spoilage.

  • III. Administrative Responsibilities
  • Ensures compliance with agency, state and federal rules and regulations governing residential habilitation.
  • Complies with the organization policies and procedures governing the use of behavior management programs for controlling maladaptive or problem behavior.
  • Complies with policies and procedures to ensure that confidentiality requirements are upheld.
  • Supports and protects the fundamental human, civil, constitutional and statutory rights of individuals and families as defined by human rights counsel.
  • Reports to work promptly and on time and does not leave work until assigned shift ends or has approval from supervisor. Observes policies/procedures governing requests for annual, sick or other leave time.
  • Abides by all other personnel policies and works closely with supervisor to ensure compliance.
  • Maintains records in the home to meet agency, state and federal guidelines
  • Makes individual related entries in daily logs that are legible, dated, authenticated by signature and positions, in ink and without use of symbols or abbreviations.
  • Maintains electronic tracking records in accordance with the individual's plan(s) to comply with program requirements.
  • Records contacts with client's family or guardian.
  • Maintains current log for approved family visitations and/or outing with friends.
  • Maintains medication records in accordance with policy
  • Maintains appropriate documentation relating to health, safety and infection control.

  • VI. Other Required Duties:
  • Maintains required trainings and certifications as required to remain in compliance with agency, state and federal guidelines.
  • Adheres to all agency policies and procedures and SCDDSN standards.
  • Must be able to lift at least 50lbs, bend, squat, stand, walk frequently throughout the shift.
