Overview:
This position will assess the patient's individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients, families, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services.

Qualifications:
- Education: RN; BSN preferred.
- Licensure: Current Registered Nurse.
- Experience: Minimum of 3 years nursing experience.
- Certifications: Current BCLS certification.
- Other: Ability to read and communicate effectively in English. Good computer skills. Team player with excellent communication and interpersonal skills with all levels of internal and external customers (patients, medical staff, support/technical staff, outside agencies, community).
- Preferred Experience: Minimum 3 years nursing.
- Preferred Other: Additional languages preferred.
