Overview:
This position is vital in the health care delivery system. This is traditionally the patient's first clinical point of contact so patient relationship, management of patient care, management of patient information, team communication, and demonstration of conduct and activities using Patient Centered Medical Home (PCMH) principles, policies and procedures is critical. Supports the vision and the mission of Su Clinica at all times. Will coordinate care for walk-in services and incoming patient/medical calls; coordinate care for Emergency Room and Hospital visits; assist provider with Durable medical equipment and Family Medical Leave of Absence forms; assess patients without appointments and/or needing medical services; Medication prior authorizations; assist medical providers as needed with patient care and procedures to include dressing changes; insulin administration; injections/immunizations; insulin teaching; glucometer calibrations and other patient education; assist and/or performs monthly vaccine stock order, monthly medication stock order, monthly supply order; chart audits.

Responsibilities:
Patient relationship: Will greet and welcome the patient in a pleasant, respectful, culturally and linguistically appropriate manner. Assures all patient questions are answered in a positive and cordial manner. Assures the patient has a clear understanding of all communications. Communicates patient's problems/complaints to the provider. Reinforces to the patients the importance of keeping appointments or communicating changes in their appointments. Asks the required questions for a proper assessment. Engage patient and their care givers in understanding their care in a cultural and linguistically appropriate manner. Management of patients care: Prepares patients for examination and treatment. Takes patient's history and vital signs. Prepares examination and treatment rooms with necessary equipment and supplies. Prepares and administers injections within the Texas Licensed Vocational Nurse scope of practice. Assists with refills, reviews all patient medications as per protocols. Initiates notes for the nurse on call or others to alert of information regarding a patient. Assists the providers as requested. May collect and process biological specimens for analysis and other lab tests. Assists with scheduling tests or treatments as requested. Conducts appropriate screening tests and follows up with Provider using Standing Orders. Performs Nurse Coordinator duties. Teaches method of health promotion and disease prevention in accordance to individualized needs. Refers patient to appropriate resources\referrals, social services, pharmacy, laboratory, etc. Verifies that patient's and family members' discharge and follow up questions have been addressed and appropriate educational materials provided and documented in the electronic medical record. Works jointly with Provider to maximize disease prevention and reinforce patient follow through with Provider's orders and instructions. Performs all other duties as assigned. Manages patient information: Takes and documents patient's history and vital signs. Performs pre-visit chart reviews for required health indicators, vaccines, labs, etc. Reviews chart under the direction of provider and obtains and places report results in charts. Makes list of anticipated needs for patients (vaccines, labs,etc.) Assures the patient information and records are current and complete. Adheres to all Health Insurance Portability and Accountability Act (HIPAA) regulations in and outside of the Clinic. Communicates patient information as requested to enhance patient services. Team communication: Actively participates in team huddles communicating workload, anticipated patient flow needs and potential opportunities to maximize patient appointments. Meets with the team to ensure that the provider's productivity and schedule is reviewed and populated throughout the day. Assists in the orientation of new employees or students. Works with unit team members as well as other clinic personnel to ensure a smooth efficient patient flow. Communicates specific patient needs and patient information to team to enhance patient care.. Demonstration of safe and professional conduct: Maintains a friendly environment for self and others. Responds to emergency situations as per training. Refrains from texting and speaking on the cell phone while on duty. Refrains from informal communication with patients and others. Fully participates in performance improvement and follows all the Clinic policies and procedures. Attends work as scheduled in a timely manner in accordance with Clinic attendance and leave policies and performs other duties as assigned. Submits professional employment documentation in a timely manner (credentialing requirements, license renewals, certifications, continuing nurse education attendance, etc). Always adheres to a professional appearance in dress and conduct.

Qualifications:
High School or GED required. Must be currently licensed as a Licensed Vocational Nurse for the State of Texas. Minimum of 1 year experience working as a licensed vocational nurse in a hospital or medical clinic setting preferred. Must be computer literate. Must be current with Basic Life Support/Cardiopulmonary Resuscitation. Bilingual (English/Spanish) required. This position requires an initial N95 mask fit testing and annual test fitting thereafter unless there is a significant change during the year that requires retesting prior to the annual fit testing.
