***$15,000 Sign-On/Retention Bonus for Qualified Candidates*** AS AN OPIOID AND SUBSTANCE USE DISORDER PREGNANCY POSTPARTUM CARE COORDINATOR (RN), you will help mothers access evidence-based treatment for substance use disorders. The Care Coordinator conducts outreach, and brief initial assessments, introduces mothers to MAT (Medication Assisted Treatment) programs and services, Drug and Alcohol services, serves as a resouce and coach for their clients and advocates for recovery. The Care Coordinator assists with access to other services such as counseling, primary care, behavioral health services, treatment facilities for recovery and other community support services for families.
