Service Coordinator/Case Manager - Early Intervention (Children Birth-3)


Developmental Disabilities Resource Center of Lakewood, CO is seeking to hire a full-time Service Coordinator/Case Manager - Early Intervention (Children Birth-3) to provide resource coordination to children (birth-3) who are enrolled in the early intervention program.

Compensation
  • $20.50-$22.55/hour DOQ.
  • 36 hours per week/4 days per week, with Friday's off.

A DAY IN THE LIFE AS A SERVICE COORDINATOR/CASE MANAGER - Early Intervention (Children Birth-3)

As a Service Coordinator/Case Manager - Early Intervention (Children Birth-3), you are responsible for the planning, coordinating, and monitoring of Early Intervention (EI) services provided to children with developmental delays and their families. This position determines eligibility for EI services and facilitates the Family Assessment and the Individualized Family Service Plan (IFSP) process with optimal participation of the family and extended IFSP team. The Service Coordinator serves as the primary contact for families from referral to the child's exit from early intervention services at age three (3). This position also informs families of their rights and procedural safeguards and how to exercise them.

QUALIFICATIONS FOR A SERVICE COORDINATOR/CASE MANAGER - Early Intervention (Children Birth-3)
  • Bachelor's degree in Early Childhood Education, Social Work or related degree in Human Services preferred.
  • Minimum 5 years of experience in case management or appropriate combination of education and experience required for applicants without a degree.
  • Case Management and experience working within the field of Intellectual and Developmental Disabilities, Early Childhood or related systems strongly preferred.
