Job Description
Community Healthcare Network, is a Federally Qualified Health Center (FQHC) serving the under served communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually.
We are seeking a Women's Health Nurse Practitioner for our Health Centers based throughout Brooklyn, Bronx, Manhattan and Queens.
Reporting to the Medical Director for Women's Health, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.
