* Job Summary
* Provide basic patient care under the direct supervision of a registered nurse. Will assist in providingactivities of daily living and comfort measures according to a verbal report for assigned patients. Assist withpatient care activities /services of a technical nature, and assists with daily unit activities and order.

* Minimum Qualifications

* High School Diploma and/or GED

* 1 year or more experience in a hospital, nursing home, home health aide environment preferred

* Working knowledge on the following Safe Lift Equipment: Viking, Sabina, Ergoslide, Hoyer lifts, andGolvo. Gaitbelts, vital signs machine, and weighing scales.

* Primary Duties and Responsibilities
* Take vital signs (temperature, blood pressure, pulse, pulse ox and respiratory rate) and report anyabnormalities to the nurse in charge of patient and record legibly on nursing flow sheets.

* Accu-check monitoring.

* Provide direct patient care such as assisting with bathing, eating, dressing, and walking patients.

* Turn and reposition bedridden patients to prevent breakdown of their skin.

* Change bed linens.

* Record amount of oral intake and measure urinary output.

* Collect specimens for tests making sure they are labeled and transported properly.

* Supply and empty bed pans.

* Transport patients and equipment as needed.

* Assist patient with ambulation.

* Follow infectious disease precautions to prevent the spread of organisms.

* Aware of patient fall prevention interventions and able to identify patients at risk for fall.

* Applies supportive binders, aces, anti-embolism stockings, SCD's and foot pumps.

* Provides sitz baths as required.

* Assist with ice pack and K-Pads when ordered and directed by RN.

* Provides post- mortem care and transporting the body to the morgue.

* Responds to patient lights promptly.

* Talks with and listens to patients and families, recognizing and reporting pertinent information.

* Perform other related duties as assigned.
