MAJOR DUTIES:
  • Demonstrate clinical competencies in such areas as age-specific care, cultural assessment, and health education
  • Demonstrate an ability to effectively function autonomously, as well as a member of an interdisciplinary team
  • Ability to demonstrate understanding of facility performance metrics that can be influenced by the programs in the work area
  • Ability to demonstrate leadership and active participation in program level quality improvement processes and initiatives
  • Ability to demonstrate use of metrics to show improvement or sustainment
  • Ability to demonstrate active participation in customer service improvement processes and initiatives for the work area
  • Ability to demonstrate leadership in meeting, reporting, and training others on assigned program(s) objectives and metrics
  • Ability to demonstrate self-direction as assigned program(s) Lead RN to operate assigned programs according to VHA and facility directives, and train others within the program to ensure succession planning

  • Demonstrate effective use of facility processes for Standard Operating Procedures, Center Memorandums, Protocols, Guidelines, Patient Education, etc with some supervisor direction
  • Consistently demonstrate effective communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers, team members, and supervisors

  • Demonstrate understanding of the facility staffing model and the need to adjust staffing according to the needs of the Veteran; this includes floating to assist other program areas when staffing is needed
  • Ability to demonstrate self-direction in: Maintaining current licensure, certifications (BLS/ACLS), competencies, and mandatory trainings. Demonstrate effective use of facility processes for Standard Operating Procedures, Center Memorandums, Protocols, Guidelines, Patient Education, etc. Schedule and present at least two educational presentation to peers regarding relevant topics per year
  • Serve as a mentor to new Nurses. Provide feedback of new employee's progress to Nurse Manager
  • Utilize VHA and community resources as appropriate
  • Educate the patient and the caregiver as needed
  • Demonstrate ability to be unit/program representative at interdisciplinary meeting as assigned by nurse manager
  • Embody patient-centered care, the "I Care Values" and demonstrate respect for both internal and external customers; demonstrate commitment and leadership in delivering and improving team-based patient care within and beyond the healthcare system. Prevent lapses in care, communicate patient requests, care needs, and changes with Provider and team on an ongoing basis
  • Work collaboratively with the core and extended PACT team, as well as with the community partners and stakeholders such as Veterans Home and Shelters to meet the needs of homeless veteran populations
  • Demonstrate advanced clinical knowledge in assessing, planning, implementing, documenting, and evaluating care for an assigned panel of patients at the OKCVA HCS: recognizes the socio-economical, age-related, gender-specific, cognitive, physical and emotional needs of the adult, geriatric, and homeless patients, while focusing on chronic disease management
  • Provide chaperone support for medical exams when needed, understand, and foster an environment that is sensitive to Military Sexual Trauma (MST). Facilitate and address immediate care needs, foster engagement in treatment plan and behavior changes geared at achieving stable housing placement
  • Coordinate care with PCP, specialty services and ancillary services; dual care coordination. Also, Telehealth and CCHT consults. Arrange for Home Health agencies, refer patients for Diabetic Eye Screening, and determine appropriateness for CCHT, OEF/OIF, and HBPC
  • Ensure timely triage and assessment of walk-in patients and/or patients with urgent care needs, timely management of clinical reminders, and ensure same-day and timely management of view alerts, and secure messaging, complete nurse encounters and note on same day but no later than 2 business days per VHA's Directive and clear clinical reminders during visit
