Job Summary

Provides leadership, consultation, research, education and patient care management expertise for specific patient care populations across the health care system. Contributes to the advancement of nursing practice and continuous improvement of patient care outcomes through involvement in community clinical support, education, research, consultation and professional advancement. The Perinatal Program Coordinator is responsive to the dynamic need of the organization, staff, community and patients within the changing health care environment.

Accountabilities

CONSULTANT

1. Consults and collaborates within and across the Greenville Health System continuum of services to direct patient care management for patients and families within the area of clinical expertise.

2. Evaluates the quality of care provided and identifies needed changes in practice within assigned areas to ensure continuous quality improvement and enhancement of patient and family care.

3. Acts as a liaison/consultant promoting inter/interdepartmental communication and collaboration

4. Coordinate and collaborate with key community groups and others to address issues to improve systems of risk appropriate care. 20%

LEADER

1. Provides direct/indirect supervision of clinical practice of licensed staff nurses and health care team members.

2. Provides leadership for performance improvement teams that have the potential for cost saving initiatives and clinical improvement in practice and work flow.

3. Assists nursing leadership in employee/department improvement activities relative to clinical practice.

4. Participates, facilitates, and /or leads system committees which determine and guide clinical policy and practice standards related to area of expertise.

5. Develops and supervises program development and participates in designing processes within the area of expertise congruent with patient/family needs and organizational goals.

6. Role models patient and staff advocacy behaviors.

7. Investigates, analyzes and leads the implementation of new skills, technology and equipment for safety and clinical/fiscal efficacy.

8. Collaborates with Director and leadership to achieve targeted financial indicators, i.e., LOS, practice changes.

9. Identifies and contributes to cost savings process improvements in collaboration with nursing leadership .25%

EDUCATOR

1. Collaborates with other health care team members to develop, implement, and evaluate quality programs to meet the needs of population(s) served in a cost effective manner.

2. Determines and develops the competency processes within the scope of expertise.

3. Collaborates with nursing leadership and staff of outlying facilities served to determine educational needs.

4. Cultivates staff skills and knowledge in the area of clinical practice, critical thinking and problem solving to promote positive patient outcomes. 25%

PROFESSIONAL DEVELOPMENT

1. Attend at least one national meeting on Perinatal Care.

2. Attend workshops or conferences to maintain knowledge, skills, and abilities.

3. Collaborate with the Neonatal Outreach Educators in the other Regional Perinatal Centers. Include a summary in progress. 5%

CLINICAL PRACTITIONER

1. Guides nursing staff in the formulation of a patient care management plan for clinically complex patients or families in collaboration with the interdisciplinary team.

2. Directs and mentors nursing staff in the development of clinical judgment to promote positive patient outcomes.

3 Assesses the quality of patient care delivery and identifies opportunities for changes in practice based on current evidence and improved patient outcomes.

4. Guides nursing staff in the integration of Plane-tree concepts, ethical standards, diversity and customer satisfaction into care delivery.

5. Implements cost effective initiatives in delivery of quality patient care.

6. Reviews variance data to identify opportunities for improvement. 25%

Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Education

Bachelor's Degree - BSN

Minimum Experience

5 years - Nursing to include 3 years in specialty area

Required Certifications/Registrations/Licenses

RN - ADVANCE PRACTICE - NLAP

Other Required Experience

MSN - Preferred

Certification by specialty board credentialing organization (ANCC) - Preferred
