Requirements:

* Current registered licensure with the State of Michigan
* Current Basic Life Support/CPR certificate
* BSN preferred
* Valid Michigan drivers license with good driving record

Responsibilities:

* Provides education, care coordination, care plan implementation, and management of Maternal Infant Health Program services to prenatal and postnatal women and infants up to one year. Services are primarily provided in the home.
* Works directly with medical providers in providing prenatal care and in developing and implementing a case management plan for each pregnant woman.
* Participates in interdisciplinary planning with social workers, dieticians, medical providers, and community health workers for meeting patient needs and health service objectives.
* Provides teaching and support on family planning, pregnancy, chronic disease, infant health and development and other medical issues as needed.
* Completes MIHP risk screening assessment on new patients, progress notes on all visits, and discharge summaries upon completion of MIHP services.
* Performs initial intake visits on prenatal and infant patients. Provides appropriate referrals to address the needs of the prenatal and/or infant patient.
* Participate in quality assurance, including chart audits, caseload reporting, etc.
* Understands and adheres to CSHS's compliance standards as they appear in CSHS's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
* Keeps abreast of all pertinent federal, state and CSHS's regulations, laws, and policies as they presently exist and as they change or are modified.
* Other duties as assigned.

Skills/ Knowledge/ Abilities:

* Basic Life Support/CPR training according to requirements of license and/or certification.
* Good communication skills, both verbal and written.

* Good leadership and organizational skills.
* Demonstrated computer and typing skills
* Sensitivity to the physical, emotional, and psycho-social needs of the patients.
* Practical knowledge and understanding of nursing techniques, methods and terminology.
* Ability to work and communicate with people of different cultures.
* Ability to establish and maintain effective working relationships with other staff members.
* Ability to exercise good judgment in appraising situations and making decisions.
* Ability to work with limited supervision and to accept and profit from constructive criticism.

Experience:

One or more years experience in community health, pediatric and/or maternal child health nursing preferred; or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job as listed above.
